MoistureShield Payment Details
Rebates issued via check to builders based upon the verifiable information provided. Checks will be sent via UPS or FedEx to the address noted on this form or submitted via email. Please allow 10-12 weeks to receive the rebate check.
Terms and Conditions
- Can only submit one rebate claim per licensed company / contractor / installer.
- Rebate is not available to homeowners. Rebate is not available to any contractor/installer that has submitted a rebate request within the last 24 months.
- Cannot be combined with any other rebate or discount on MoistureShield products.
- Applies to MoistureShield products* purchased in the United States between January 1, 2024 - December 31, 2024.
- All completed rebate forms and supporting documentation must be submitted or postmarked no later than December 31, 2024.
- Invoices must show company name, date of purchase, product description / brand, quantity and purchase price. Failure to provide this information could result in the delay or denial of the rebate.
- Each rebate submittal shall be subject to review and approval by MoistureShield and shall be subject to the terms and conditions of the applicable rebate.
- A completed and signed W9 (October 2018 Version) must accompany this rebate submission.
- For residents of Canada, a completed and signed W-8BEN (Individuals) or W-8BEN-E (Businesses) must accompany this rebate submission.
*MoistureShield products include Elevate, Vantage, Meridian, and Vision decking lines as well as MoistureShield hidden fasteners, railing, and lighting.
