  1. Submit a copy of all invoices, documenting the number of MoistureShield boards purchased for each deck.
  2. Provide the homeowner name and complete address of each deck installation
  3. Provide the following information
    • Contractor Name
    • Complete mailing address
    • Phone number
    • Tax ID#
    • Key contact or contacts
  4. Send the above information directly to AERT. Must be postmarked no later than April 29, 2011
